Meet Mitchell Cabisudo, MD
Physician-Directed MeRT and TMS Treatment
Dr. Mitchell Cabisudo is a double board-certified child, adolescent and adult psychiatrist and the physician directing MeRT and TMS treatment at Advance TMS.
For more than a decade, Dr. Cabisudo has worked with children, adolescents and adults facing psychiatric and neurological challenges. His interest in advanced, non-invasive treatment options led him to bring Transcranial Magnetic Stimulation (TMS) to patients in the Hudson Valley, Manhattan and Long Island and, subsequently, to incorporate the more individualized MeRT approach.
Dr. Cabisudo’s philosophy is based on a straightforward idea: patients who share the same diagnosis do not necessarily have identical patterns of brain activity or respond to treatment in exactly the same way.
MeRT provides additional information that can be considered when developing an individualized treatment protocol.
At Advance TMS, patients receive physician-directed care supported by a clinical team experienced in TMS, MeRT and brainwave-guided treatment.
What Is MeRT Therapy?
MeRT stands for Magnetic e-Resonance Therapy. It is an individualized approach to repetitive Transcranial Magnetic Stimulation (rTMS) that incorporates information obtained from brainwave testing.
Traditional TMS uses magnetic pulses to stimulate targeted areas of the brain. MeRT adds an additional layer of personalization by beginning with an assessment that includes a quantitative electroencephalogram, or qEEG, and other clinical information.
The qEEG records electrical activity in the brain. This information is analyzed to identify patterns that may help the clinical team develop an individualized treatment protocol.
Treatment itself remains non-invasive. Magnetic pulses are delivered through a treatment coil positioned outside the head. There is no surgery, no implanted device and no anesthesia.
The MeRT Process
1. Brainwave Recording
A qEEG records the brain’s electrical activity and provides information about individual brainwave patterns.
2. Data Analysis
The brainwave information is analyzed along with relevant clinical information to help identify patterns and guide treatment planning.
3. Personalized Treatment Protocol
Based on the assessment and physician review, an individualized magnetic stimulation protocol may be developed for the patient.
4. Progress Monitoring
The patient’s clinical response can be followed during treatment, and additional assessments may be used when appropriate to help evaluate progress and guide treatment decisions.

How Is MeRT Different From Standard TMS?
Both traditional TMS and MeRT use non-invasive magnetic stimulation. The primary distinction is the way the treatment protocol is developed.
Traditional TMS protocols generally use established treatment parameters based primarily on the condition being treated and the targeted area of the brain.
MeRT incorporates qEEG brainwave information and additional analysis as part of developing a more individualized stimulation protocol.
Traditional TMS
• Uses non-invasive magnetic stimulation
• Targets specific areas of the brain
• Does not require medication or anesthesia
• Uses established treatment protocols for FDA-cleared indications
MeRT
• Uses non-invasive magnetic stimulation
• Incorporates qEEG brainwave assessment
• Uses individual brainwave information in treatment planning
• Allows the protocol to be individualized based on the patient’s assessment
Neither approach should be considered automatically appropriate for every patient. The decision to recommend traditional TMS, MeRT or another treatment should be based on an individual clinical evaluation.

Who May Be Evaluated for MeRT?
Advance TMS evaluates children, adolescents and adults seeking options for a variety of psychiatric, neurological and cognitive concerns.
Patients and families frequently contact us to discuss conditions including:
Depression
TMS has established FDA-cleared applications for Major Depressive Disorder. Patients who have not experienced sufficient improvement with medication or who have difficulty tolerating medication may wish to discuss whether TMS is appropriate for them.
Obsessive-Compulsive Disorder (OCD)
Certain forms of TMS have FDA clearance for the treatment of OCD. The appropriate treatment approach depends on the individual patient and the equipment and protocol being used.
Post-Traumatic Stress Disorder (PTSD)
Patients with PTSD may contact Advance TMS to discuss MeRT and other treatment options. Applications of MeRT/TMS outside specific FDA-cleared indications should be discussed with the treating physician.
Autism Spectrum Disorder
Families seeking information about MeRT for a child or adult with autism can schedule an evaluation to learn more about the technology, current evidence, potential limitations and whether further assessment may be appropriate.
Brain Injury and Concussion
Changes in concentration, memory, sleep, mood and other functions can occur following traumatic brain injury or concussion. MeRT uses qEEG information as part of an individualized assessment and treatment-planning process.
Other Psychiatric and Neurological Concerns
Advance TMS also evaluates patients with other psychiatric, cognitive and neurological concerns on an individual basis.
Important: FDA clearance applies to specific devices and treatment indications. The use of TMS or MeRT for other conditions may be considered off-label. An evaluation with a qualified medical professional is necessary to determine whether a particular treatment may be appropriate.

What to Expect When You Contact Advance TMS
Choosing a treatment for yourself or someone in your family can involve a lot of questions. We want patients to understand both the potential benefits and the limitations of the treatments they are considering.
1. Initial Consultation
The process begins with a conversation about your concerns, medical and treatment history, symptoms and goals.
2. Clinical Evaluation
Our clinical team reviews your information to determine whether further evaluation for TMS or MeRT may be appropriate.
3. Brainwave Assessment
When MeRT is being considered, qEEG testing may be used to record and analyze individual brainwave activity.
4. Physician Review and Treatment Planning
Dr. Cabisudo and the clinical team review the available information and determine whether treatment is appropriate and what type of protocol should be considered.
5. Treatment and Follow-Up
Treatment is performed on an outpatient basis. Progress is monitored throughout the course of care, with treatment decisions based on the patient’s individual response and clinical needs.
Does Insurance Cover MeRT?
Insurance coverage depends on the condition being treated, the type of TMS being provided, the patient’s insurance plan and the applicable coverage requirements.
Traditional TMS for certain FDA-cleared indications, including Major Depressive Disorder, is covered by many insurance plans when medical-necessity requirements are met.
MeRT and applications of TMS for conditions outside an insurer’s covered indications may not be covered and may require self-payment.
Because benefits and authorization requirements vary considerably, the Advance TMS team can help patients understand the insurance-verification process before treatment begins.
Patients should never assume that a treatment is covered solely because their insurance plan covers other forms of TMS.

Frequently Asked Questions About MeRT
What is MeRT therapy?
MeRT, or Magnetic e-Resonance Therapy, is an individualized approach to repetitive Transcranial Magnetic Stimulation that incorporates qEEG brainwave information into assessment and treatment planning. Magnetic stimulation is then delivered non-invasively according to the prescribed protocol.
Is MeRT the same as TMS?
MeRT uses TMS technology, but the treatment-planning approach differs. MeRT incorporates qEEG brainwave information and additional analysis to help individualize the stimulation protocol.
Is MeRT invasive?
No. Magnetic stimulation is delivered from outside the head. Treatment does not involve brain surgery, an implanted device or anesthesia.
Who provides MeRT treatment in New York?
Advance TMS provides MeRT treatment under the direction of Mitchell Cabisudo, MD, a double board-certified child, adolescent and adult psychiatrist. Advance TMS has New York locations in Manhattan, Great Neck on Long Island and New Windsor in the Hudson Valley.
Is MeRT FDA cleared?
This question requires an important distinction. FDA clearance applies to particular medical devices and specific treatment indications rather than broadly establishing every use of a technology for every condition.
TMS devices have FDA-cleared indications for certain conditions. Other applications may be considered off-label. Patients considering MeRT should discuss the regulatory status of the equipment and proposed treatment for their particular condition with their physician.
What conditions are evaluated at Advance TMS?
Advance TMS evaluates patients seeking care for depression, OCD, PTSD, autism, brain injury and other psychiatric, neurological and cognitive concerns. Whether TMS or MeRT is appropriate depends on the individual patient, and not every application has the same FDA status or level of clinical evidence.
Does insurance pay for MeRT?
Coverage varies. Many insurance plans provide coverage for traditional TMS for certain FDA-cleared indications when eligibility and medical-necessity requirements are satisfied. MeRT and treatment for other conditions may be self-pay. Advance TMS can help patients investigate their individual benefits.
How long does MeRT treatment take?
Treatment schedules vary according to the patient, condition, protocol and clinical response. The Advance TMS team will explain the recommended schedule after evaluation rather than assuming that one treatment schedule is appropriate for everyone.
Are there side effects?
TMS is generally considered a well-tolerated, non-invasive treatment, but side effects and risks are possible. The specific risks depend in part on the treatment protocol and the patient’s medical history. These should be discussed with the treating clinician before beginning treatment.
How do I know whether MeRT is right for me or my child?
The best starting point is an individual consultation. A diagnosis alone does not determine whether someone is an appropriate candidate. Medical history, symptoms, previous treatments, goals, potential risks and the evidence supporting treatment for the particular condition should all be considered.
